How do I obtain a Public Pet Rehoming Permit  

In order to make the process convenient for the applicant we have provided three required forms.  Once the forms are completed, the application package can be submitted electronically via e-mail or fax.  All applications will be processed within five business days. 

  1. There is no fee for the Public Pet Rehoming Permit.
  2. Please complete the Public Pet Rehoming Application, print and sign.
  3. Print off the Public Pet Rehoming Health Certificate. This form must be completed and signed by a licensed veterinarian.
  4. The Merchant/Agency Authorization for Public Pet Rehoming form must be transferred onto official letterhead and signed by an authorized agent for the business or agency. 
  5. Submit the signed application, signed Public Pet Rehoming Healthy Certificate and the signed Merchant/Agency Authorization to Mesa County Animal Services via e-mail to or fax to 970.245.5315. 
  6. All applications will be processed within five business days.
  7. Once approved, the applicant will be notified and can pick up or request that we mail the permit to the address on the application.
  8. The permit is good for 14 days and can be renewed one time for up to an additional 14 days. 
  9. The permit must be displayed within six feet of the enclosure housing the animals and must be visible at all times.
